Between them they run the clinic, the research office, the publishing division, the teaching, and the finances — which is honest about the scale of the organisation, and honest about the fact that in an organisation this size, the people who decide are also the people doing the work.
That concentration is a strength and a risk, and we treat it as both. None of the four may sit on the Institutional Review Board, hear an appeal, or decide a complaint about a decision they took.
